The Supreme Council has revised the State Council Service Program Awards Entry Form (#STSP). We have revised New Hampshire State Council's on-line submission for to reflect the changes to Form# STSP. Section 3, which is where the council provides documentation of their program submission, was revised. The change to the form is outlined below.

- Councils provided a brief description of the purpose and goal of the program being submitted in section 3a.
- In addition, the council would need to provide the documentation for the following:
3b) Whom does this project benefit? 3c) What problem or need did this project resolve? 3d) Why did the council select this project? 3e) Describe the success of the project.

The new form contains a single section for the councils to document the program that is being submitted. In section 3, the councils must describe the program in full detail. The following additional requirements were added to the form.
- Programs must be organized by the council or involve significant participation by council members to qualify.
- Programs must also engage members by enhancing faith and spirituality, serving a charitable purpose, or a combination of the two.
- Program descriptions should reference how they meet these criteria.
- Along with the description of the program, provide the program's purpose, goals, accomplishments and why it deserves to win the award.
